Name* First Last Email* Consent* I have read and agree to the terms of the Saltaire Recreation Policies below.2021 RECREATION PROGRAM POLICIES Age Group Policy Placement in age groups will be determined by the participant's school grade starting in September, and in no instance may a child participate in an activity until he/she has reached the age of 4. Refund Policy 50% Refund of any Program Session (2-Week Session for Camp, 4-Week Session for Junior Lifeguarding and 1-Week Session for Library) payment will be given if the session is canceled in writing no shorter than 1 week before the session starts. If the Camper or a direct-contact family member has a confirmed positive COVID test before or during their session, partial refunds will be considered in consultation with the Recreation Medical Director on a case-by-case basis. Use of Image or Likeness I grant the Saltaire Day Recreation, its representatives and employees the right to take photographs of my child in connection with his or her participation in the recreation, and I further authorize its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that the Village or Saltaire may use such image of my child with or without his or her name for any lawful purpose, including for publicity, illustration, advertising and Web content. Medical Coverage and Waiver with Covid-19 Waiver The Recreation Program provides limited accident insurance coverage for each child during participation in recreation activities. It is not in any way a substitute for family medical coverage and the Village strongly encourages that each participant should maintain their own sufficient medical coverage. I as the parent or legal guardian give permission for emergency diagnostic and therapeutic or surgery as may be deemed necessary for my child by the medical personnel in charge of care at the time; and I will also provide information concerning my child's medical condition to the responsible officials when deemed necessary. My child is in good health and has no COVID-related symptoms (described later herein) today and has my permission to enroll and participate. My child has no previous sickness, illness, disease or bodily injury which is in any way a constraint or restriction to his/her participation in the Saltaire Recreation Program. Covid-19 Waiver Language Covid-19 and its variants are extremely contagious and are believed to spread mainly from person to person contact and by aerosol transmission if in close proximity. As a result, federal, state and local governments and health agencies recommend safety measures, and in some cases have prohibited the congregation of large groups of people. The Village of Saltaire has implemented reasonable preventive protocols to reduce the spread of Covid-19 in the Village. Despite the measures planned on being taken to mitigate the risk of transmission of Covid-19 in the Saltaire Recreation Program, there are inherent risks that your child and or your family may become infected with Covid-19 due to your child’s participation in the Saltaire Recreation Program. The Village of Saltaire cannot guarantee that you or your child(ren) or other family members will not become infected with Covid-19 as a result of your child’s participation in the Recreation Program. Your child’s participation in the Summer Recreation Program could increase the risk of your child, you, or your family members contracting Covid-19, and by allowing your child to participate in the Saltaire Recreation Program you understand, acknowledge and voluntarily assume these risks on behalf of yourself and your child. I understand that my child’s presence in the camp may increase the risk of infection of me, my child our family members with Covid-19 or its variants, I acknowledge the contagious nature of Covid-19 and its variants and voluntarily assume the risk that my child or I or other family members may be exposed to or infected by Covid-19 by my child’s participation and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for injury to myself or my child(ren) or family members including, but not limited to, personal injury, disability, and death, illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my child’s participation in the Recreation Program. On my behalf, and on behalf of my child(ren) I hereby release, covenant not to sue, discharge, and hold harmless the Saltaire Recreation Program and the Village of Saltaire, its employees, agents, officials and representatives, from any claims, including all liabilities, claims, actions, damages, costs or expenses based on the actions, omissions, or negligence of the Village of Saltaire and its Summer Recreation Program, its employees, agents, officials and representatives, whether a Covid-19 infection occurs before, during, or after participation in the Summer Recreation Program. I agree that to the best of their ability my child will follow all safety guidelines required by the Summer Recreation Program in effect at the time of attendance and practice proper hygiene at all times during participation in the Summer Recreation Program. I further acknowledge that my child’s failure to comply with the requisite safety guidelines may result in my child's expulsion from the Summer Recreation Program. Mandatory Health Screening and Covid Policy The Saltaire Recreation Program reserves the right to require one or more of these screening and testing protocols, the implementation of any of which will depend on the local conditions and recommendations of the Suffolk County Department of Health at the start of the Camp Session. Participants and Staff will be required to follow a daily pre-screening protocol before contact with fellow staff or program participants. Anyone who is sick with anything should stay home, and if identified during the activity will be instructed to go home. Anyone who stays home, is sent home, or has symptoms of an illness, or who has a family member who has symptoms of an illness must be tested and/or meet current quarantine periods prior to returning to any activity or work. Morning health and/or temperature screening of all participants and staff before joining their group. If anyone feels ill at recreation activity (with anything) parents will contacted and child will be sent home ASAP. The Village will take the following action if there is a COVID-19 Positive Diagnosis with the Camp, which would include Staff, Campers and Parents. If a participant or staff member reports they are COVID-19 positive, VOS will maintain the confidentiality of the individual at all times while mitigating the situation.
